Well the kernel is the correct one...
I would at all costs avoid UV, it's not usefull at all, will save extremely little battery, so little, you couldn't measure it anyway, but it opens the door wide open for instability.
But you also tested with stock voltage, so it's not that.
Have you tried with stock settings, ONDEMAND and mpdecision, just in case ?
Also, for the efs issue, I've had that exactly once, the day I switched to TWRP.
I've reverted to Chenglu's CWM, ever since, not happened again, could be unrelated, but well ...
